Rose Kadzere scored the decisive goal in the 79th minute, completing Malawi's comeback and moving the tournament debutants closer to an unprecedented continental title.
The victory also secured Malawi one of Africa's four automatic places at the 2027 FIFA Women's World Cup in Brazil, marking another historic milestone for the women's national team.
Malawi had already stunned the continent during the group stage, defeating defending champions Nigeria and Egypt before finishing top of Group C on their maiden appearance at the tournament.
Their progression to the semi-finals now places them among the few debutants in Women's AFCON history to reach the last four.
Ghana, meanwhile, suffered a major setback in their bid to return to the Women's World Cup for the first time since 2007.
The Black Queens still have an opportunity to qualify through the African play-off, where they will face Ivory Coast, with the winner advancing to the intercontinental play-offs.
Ghana made the brighter start in the quarter-final and took the lead after just seven minutes when Chantelle Boye-Hlorkah finished clinically.
Malawi responded almost immediately, with captain Temwa Chawinga restoring parity in the 12th minute to reignite her side's hopes.
The contest remained fiercely competitive after the break, with Ghana briefly thinking they had regained the lead through Doris Boaduwaa. However, the goal was ruled out following a VAR review.
